

HF's 'Connections for HOPE' outreach program
Our homeless outreach program is a vital component of our mission to supporting individuals that require basic need assistance as well as substance use disorder and mental health challenge resources. The Connections for Hope team can be found in the community Monday through Friday engaging with our unhoused population and doing our part to make a positive impact for our brother and sisters.

Mondays 9:00am - 12:00pm
Life Center

Every Monday, we collaborate with the Life Center to address immediate needs and offer development opportunities for individuals seeking support. During their time there, the team interacts with both established groups and individuals while also building new connections.
Tuesdays 10:00am - 2:00pm
Giving Heart
4358 S Broadway, Englewood, CO 80113
Every Tuesday, the team is present at Giving Heart to provide support in various areas, including Community Resources, Lunch services, the Mailing Site, ID assistance, the Clothing Resource Room, the Computer Lab, and activities such as Movie Tuesday.


Wednesdays 9:00am - 12:00pm
Bridge House Transition Center
6507 S Santa Fe Dr, Littleton, CO 80120
On Wednesday the team is at the Bridge House. The Bridge House is a 16 bed acute treatment unit (ATU). The ATU is for ages 18 and up and treats clients who need an inpatient stay due to a psychiatric emergency. The ATU offers care that’s not as intense as an inpatient stay but is a higher level of care than a crisis stabilization unit.

Outreach Team
These are the individuals in the mud - the group of peers doing the challenging and impactful work with the most vulnerable members of our community. Coupled with a skillset and knowledge of how to navigate the system of resources currently available to those they serve; Compassion, Empathy and Resolve are the make-up of each of their character. They come from a place of lived experience. That lived experience helps foster trust amongst those they outreach. Through their coordinated efforts, individuals are assisted in obtaining resources not limited to:, Food, Housing, Hygiene, Identification Needs, Transportation Assistance, Clothing, Peer Support, Informational Support, Emotional Support, Inpatient Rehabilitation, Outpatient Rehabilitation.
