On July 25, 2025, we came together for a Community Appreciation BBQ that reminded us why we do this work. We shared grilled favorites, swapped stories, and celebrated the everyday kindness that keeps our community strong. Most importantly, we honored the courageous neighbors we serve through Connections of HOPE—HF’s outreach program walking alongside people who need support with basic needs, substance use challenges, and mental health.
About Connections of HOPE
Connections of HOPE is HF’s community-based outreach initiative. Our team is out in the field Monday through Friday, meeting people where they are to offer compassionate support and practical resources. We help neighbors navigate systems, access essentials, and take next steps toward stability.
What we provide (and coordinate):
Food, clothing, and hygiene items
ID and documentation assistance
Transportation support
Peer support and emotional support
Referrals for mental health care
Substance use treatment (inpatient and outpatient)
Information, advocacy, and warm handoffs to partner services
Our approach is rooted in lived experience, empathy, and resolve. Trust is the doorway—relationship is the work.
