In consideration of being permitted to participate in the 2026 Hornbuckle Foundation Community Carnival (the "Event") on 9/13/26, including inflatable rides, obstacle courses, water slides, carnival games, and all other activities offered on-site, I, the undersigned parent/guardian and/or adult participant, acknowledge and agree to the following on behalf of myself and any minor child(ren) named below:
1. Assumption of Risk
I understand that Event activities carry inherent risks of injury — including but not limited to falls, collisions, strains, and other physical injury — and I voluntarily assume all such risks on behalf of myself and/or my minor child(ren).
2. Release of Liability
On behalf of myself, my heirs, and my minor child(ren), I release and hold harmless the Hornbuckle Foundation, its board members, staff, volunteers, and sponsors, Ken Caryl Ranch Community Park, and the equipment provider, from any and all claims, liabilities, damages, or causes of action arising from participation in the Event, except in cases of gross negligence or willful misconduct.
3. Rules & Wristband Acknowledgment
I understand that every child must wear a wristband to use the inflatable games, and I agree that my child(ren) will follow all posted rules and the instructions of Hornbuckle Foundation staff and volunteers.
4. Medical Authorization
In the event of injury or medical emergency, I authorize Hornbuckle Foundation staff/volunteers to seek emergency medical treatment for my child(ren) if I cannot immediately be reached, and I accept financial responsibility for any resulting medical costs.
5. Photo & Media Release
I grant the Hornbuckle Foundation permission to photograph or record myself and/or my child(ren) during the Event and to use such images for promotional or fundraising purposes, without compensation.
6. Acknowledgment
I have read this waiver, understand its terms, and sign it freely and voluntarily.
